| Bid Acceptance Agreement Form | Must be submitted by each new member within 48 hours of accepting a bid (invitation to membership) |
| Change in Status Form | Must be submitted within 48 hours of a membership status change |
| Programming with Alcohol Event Registration Form | Required for each event in which alcohol will be present; submission of form does not guarantee the event will be approved. See Policy. |

* Please note that the supplemental meal plan is ONLY available to Greek students residing a residence hall who contract for more than seven (7) meals per week at their chapter house; chapter meal plans must be approved by the University and comparable to that of the University Dining Plan for members to be eligible for the waiver
